Requirements Who We Need The Community Health Educator works to grow and sustain Healthier Texas programmatic work that contributes to saving lives by reducing the instance of preventable diseases while increasing access to health for all Texans. The Community Health Educator will serve as a trusted resource and the broader community, supporting the Health + Healing Hub project to foster a culture of health within the Fort Worth/Tarrant County area. This position focuses on providing individual and community-based health interventions through outreach, providing health education, and advocacy. The Community Health Educator will work to cultivate relationships and build partnerships with local community organizations (including faith-based organizations), businesses, and other stakeholders to disseminate health information and recruit program participants for engagement in health programming. As a key liaison between the Community Health Educator must reside within the Forth Worth/Tarrant County area and will be required to participate in and complete a 16-week Community Health Worker training. This is a hybrid position that is housed within a faith-based organization. This position is grant funded through December 31, 2026. Essential Functions & Responsibilities Program Planning and Implementation (50%) Facilitate culturally appropriate workshops and health promotion activities tailored to the needs of the 76104 and 76105 communities. Collaborate with program staff to develop and refine health programming that aligns with participant needs and program goals. Assist in the creation of educational materials, resources, and tools for program participants and wider community. Monitor participant progress and provide follow-up support to ensure continued engagement in health improvement activities. Maintain updated participant records with plans, notes, appropriate forms, or related information. Identify barriers to participation and recommend strategies to improve accessibility and inclusivity of health interventions. Advise participants and community members on issues related to improving general health, such as diet or exercise. Outreach and Relationship Management (30%) Actively engage with community stakeholders, including local businesses, nonprofits, and civic organizations, to disseminate health information and build support for Healthier Texas Develop and maintain an outreach calendar to ensure consistent and strategic engagement within partners and community members. Serve as a trusted resource within the community to answer questions and guide individuals in accessing health services Collaborate with faith leaders and other influencers within the church setting to integrate health-focused messaging into existing community networks. Strengthen connections with culturally specific organizations to ensure outreach strategies are inclusive and representative of community demographics. Data Collection and Reporting (20%) Lead data management process execution for their caseload. Identify and support the collection of partner and community member testimonials that demonstrate our program's impact Ensures data accuracy within programs? Support integration of community feedback into program model Assist in the evaluation of data to improve program effectiveness and efficiency?? Additional reporting as needed??
