Title: Project Manager, Quality Improvement & Outcomes
Location: 4822 N Broadway, IL 60640
Schedule: Hybrid, Monday to Friday (8:30am-5:00pm)
Summary:
The Project Manager, Quality Improvement & Outcomes partners with program staff, management and leadership in HHCS’ programs Initiatives to ensure data quality, successful outcomes report, and process improvement as it is related to data collection/reporting. This position will serve as the Initiative lead in the efficient and effective use of the agency’s Case Management System (CMS). In consultation with the Director of Quality Improvement and the Director of Data and Information Management, sets the strategic direction for system improvements. The position will be responsible for developing and maintaining effective tools such as training, procedures, and workflows to maximize programs’ use of CMS.
The Project Manager, Quality Improvement & Outcomes provides thought-leadership to Housing, Asset Building, Travelers AID, and RICS programs while leveraging industry knowledge and best practices to promote the mission and vision of Heartland Human Care Services. Essential Functions:
Duties & Responsibilities
- Supervises staff and/or interns/volunteers.
- Responsibilities include interviewing, hiring, training, mentoring and developing employees; planning, assigning, and directing work; and all other aspects of performance management.
- Conducts supervisory responsibility in accordance with organizational policy; adheres to professional standards as outlined by governmental bodies, HHCS accreditation and funding sources.
- Data management and reporting: Coordinates or is responsible for all data management, reporting, dashboards and other reporting areas, both internal and external (i.e. to funders).
- Supports programs in funder monthly/quarterly reporting and oversees all internal quarterly/annual reporting for the HART Initiative programs.
- End User Support: troubleshoots basic technical problems related to data entry and reporting within database systems used by the programs’ Initiative, including adding, removing and otherwise managing user accounts for appropriate data systems.
- Program Support: Assists in the creation and maintenance of data workflow plans and relevant program policies and procedures pertaining to the data collection and reporting system(s), as necessary for the programs’ Initiative
Quality management & quality assurance:
- Monitors data quality and if needed, works with the program Initiative leadership staff to develop and implement strategies and systems to improve data quality.
- Works with Program Managers to appropriately prioritize and implement quality improvement activities or processes designed to address data quality deficiencies and/or improve outcomes.
Data-driven decision-making and quality improvement:
- Coordinates, provides and interprets person specific, program specific and initiative-wide performance data to support quality improvement, program evaluation, proposal development, internal and external reporting, strategic partnerships and communication efforts. These activities are focused on measuring outcomes consistent with organizational goals and values, and with funding and Heartland requirements or best practices.
- Participates in HHCS programs’ Initiative management meetings, All-Staff meetings, and program specific meetings, as appropriate, to provide, interpret and discuss data and program outcomes, and as a member of the programs’ management.
- Develops and facilitates in-service training related to program performance as well as data collection systems and outcomes analysis.
- In consult with Director of Quality Improvement, conducts presentations, trainings and workshops to facilitate data-driven decision-making, quality improvement, and programmatic goals, outputs and outcomes.
- In consultation with the Director of Quality Improvement, primary liaison with HHCS’s Social IMPACT Research Center staff, regarding data interpretation and analysis activities that involve the Initiative’s programs.
- In consultation with the Director of Quality Improvement acts as a liaison between the Programs’ Initiative and its Case Management System (CMS) software administrator, CMS Analyst, and Director of Data Analytics and Database Administration
- In consultation with the Director of Quality Improvement provides guidance and direction on program functionality to the CMS Administrator and CMS Analyst
- Assists in the development of standard and custom reports in CMS for use by executive and program management.
- Performs other duties as assigned.
Supervisory Responsibilities (if applicable):
Qualifications:
- Bachelor’s Degree and at least (3) three years of relevant experience (data management/analysis, social services grants management, etc.) required.
- At least (1) one year of management and/or supervisory experience required; two years or more preferred.
Knowledge/Skills/Abilities (K/S/A):
- Language Skills: Excellent oral and written communication skills. Strong interpersonal skills required. Demonstrated writing skills and competency in quantitative analysis required. Ability to write reports and procedure manuals. Ability to effectively present information and respond to questions from groups of internal/external colleagues (direct service staff, management, and senior leadership), clients/participants, and the general public.
- Computer Skills: Advanced knowledge in Microsoft Office especially Word, Excel and Outlook, is required; significant experience with data entry and reporting functions of databases and data management systems/platforms; and other software routinely used by Heartland Human Care Services. Experience with analytic data systems preferred.
- Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Familiarity with fundamentals of statistics, analysis, and probability preferred.
- Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Work Arrangement:
This role is a “hybrid” work arrangement. It is expected this person would be able to meet for “in person” meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.
