We're on the lookout for a Full-Time Social Services Assistant to join our warm, resident-focused care team. You'll be an essential part of making our residents feel seen, supported, and heard throughout their stay.

What You'll Do:

• Assist with admissions, discharges, and resident care plans
• Support residents and families in navigating care transitions
• Help coordinate appointments, community resources , and family communication
• Advocate for residents' rights, dignity, and emotional well-being
• Maintain documentation in accordance with regulations and facility policies
• Partner with the Social Services Director and interdisciplinary team to provide the best care possible

What We're Looking For:

• A warm, empathetic communicator with a team-oriented mindset
• Organized and reliable with solid time-management skills
• Previous experience in social services or healthcare setting is a plus
• Strong desire to support residents' emotional and social health
