Alzheimer's and dementia . We're currently seeking a compassionate, detail-oriented Social Services Assistant to support our residents and families through care planning and transitional support.

Position Details:
Part-Time Schedule
Starting at $17/hr
Located in a warm, team-centered environment

Key Responsibilities:
Assist with resident admissions, discharges, and transfers
Help develop and update individualized care plans
Support residents and families through emotional and social challenges
Maintain accurate documentation and communication with care teams
Advocate for resident rights and well-being
Coordinate with community resources and services as needed

Ideal Candidate Qualities:
✔️ Empathetic and patient, especially with memory care residents
✔️ Strong organizational and interpersonal communication skills
✔️ Experience in long-term care or healthcare setting preferred
✔️ A team player with a heart for service
